User Template in Zoho Sign / Form + Z Desk

Hi Team, I would like to create a form in Zoho Sign or Zoho Form to send to the final user sign;
It's a term of responsibility to register some requests from the final user to open some specific ports to internet, and Would like to create a "map" of ports requested by users and the term of resposability signed.

The propouse is attach this informations directly in the tickets.
Is it possible to use Zoho Sign (templates) to this proposal?


I'll control some requests of firewall rules as shown below (example) and I need to send a doc or form to the customer, sign (agree)
