Why does the Send mail application keep braking??!!

Why does the Send mail application keep braking??!!

I built a Send email on success script for TaskTrackerYC and it keeps braking!!!! The script is correct but it stops sending emails and the "on success message" gets replaced with the default.

Am I doing something wrong or is the system unreliable?
