2 areas of Help with web to lead forms - getting email notification of a form submission & a check box issue

Hi! Our web to lead form is working pretty well.

We have two issues:

Getting the notification to my email that someone has submitted a form. It does not work. I have reconfigured the form I don't know how many times...

We have a check box if someone wants our brochure. However, it does not populate the field in the lead. 

Any tips along these lines would be greatly appreciated!
