A report for leads created by source (hopefully ediatable) for a specific time frame

Can you help? i need a report that shows all my leads for a specific time frame (i.e. last month July 2016 etc). I need to be able to filter on lead source and it needs to include leads that have already been converted.

I may generate 50 leads in July, some of them convert during july and some may convert in september (or even later) but i need to be able to generate a report that shows me all of those leads, i need to see the lead source too.

Can someone wither show me how to do this or create a report for me. I know there is a default lead conversion report but this is no good as i cant edit it.

Please help
