Activity Report showing deleted tasks

I just created a report to show Activities of my Accounts, but discovered that previously deleted Activities are showing up in the report. In fact, when I click on them in the report, I get the following error:

This action cannot be performed.

The record you are trying to access has been deleted. To restore access, remove the record from the Recycle Bin.

How can I exclude such deleted items from a report? Shouldn't they be excluded by default?

Also, how can I filter a report to exclude Closed Activities as well?
