Add Custom "Notes" Field?

Hello,

What we want to do is create a custom "notes" field to capture "Value Added Services" discussed with the account. While it sounds simple, we want to have the date automatically assigned along with the text that describes the value added discussion. Our purpose is to use this in annual account reviews, which is why we want this to "reside" with the Account.

Any suggestions?
