Adding text as part of the form page

Adding text as part of the form page

I am creating a form.   

I want the form to look like this:

Already registered?   SIGN IN below

      User Name
      Password
                                Forgot password?    (this would link to a different page)

Question.   I know how to create the user name and password fields.

how do you just add text as part of the form?
How would add a hyperlink to the text?
