Authentication issues on Safari , 2 factor authentication not working on SMS and Voice.

Safari Version 9.0.1 (11601.2.7.2) on Mac OS X 10.11.1 reloads login page continuosly.
Login on Safari now never succeds .
It shows (briefly) "reload page" up to a few instants ago.
 
SMS notifications and voice call for 2 factor authentication never get delivered , had to reset TFA for my user from administration account, and select Google authenticator. SMS and voice call don't work, not even as backup for two factor authentication .

I eventually managed to login using Firefox.

During the day I was asked the confirmation code via sms several times, too many , even if I had set the "remember this browser as secure".
