Auto Notification to call a potential 5 days after creating the potential.

I have created a task rule for me to call a customer 5 days after submitted a potential but its not working correctly instead it wil send the notification to me as soon as I i have created the potential.

What i have in there seem to relate to the Due Date field, which is not important to our business and don't use it and only select the date at the end of the month the potential was created in, as is it is a mandatory field. 

I'm not sure if Alert will do what i require, i just want it to remind me to call a customer 5 days after potential is created that is all.
