Auto upload completed forms to contacts

Hi everyone,
I'm trying to make a form that can be auto saved to a contact in Zoho as a PDF attachment. This way I can have our team fill out the form when meeting with a client then the form auto uploads against the contact in Zoho. I'm open to using any form builder but we already use Jotform for other things and it seems to work well.

So a suggested automated workflow would be:

1) User completes a form in Jotform (or similar)
2) Form is exported to PDF
3) Zapier (or other automation software) uploads it to the correct contact in Zoho.

Any ideas?
