Autoresponder Chain Not Showing up in Email Records

I set up an autoresponder that sends an email 1 day, 4 days and 9 days after lead creation time. The 9 day email contains a survey. See below for the autoresponder setup:
Then, if you look at a sample lead from more than 9 days ago, you see this:

So you can see the 1 day and 4 day (as well as a separate email campaign we've been sending) but no 9 day email. The reason I know something is wrong is that the 9 day email contains a survey that this person filled out! So I know she got the email but it isn't showing up.
