Bug: WYSIWYG table row and col add and delete control is missing

Some time in the last few weeks, the tiny control that appears when editing tables, to allow adding/deleting rows and columns, has stopped appearing.

That control was always a bit fiddly, but quite useful, particularly for adding or deleting many rows at a time.  The same action using the right click menu does work, but is quite slow.

I am reporting this in case the disappearance of this control was unintentional.
