Campaign Lookup Field doesn't work in the convert to Lead screen

Campaign Lookup Field doesn't work in the convert to Lead screen

Problem:  When converting a lead to a Contact/Account/Potential - The Campaign Source Lookup Field doesn't work...it is blank.

Steps taken:

1) Create a lead and save.
2) Click Convert Lead
3) The next screen - allowing for Account/Opportunity choices etc...The last field "Campaign Source" is blank
4) The lookup on campaign source comes up blank - even though I have active campaigns created in the campaign tab.
