I am trying to use a Web to Bug Form with the following settings to file a bug in Zoho Projects. I have tried using '*' in the the Form/Web Page URL field and flag as internal/external. On submitting the form - the webpage redirects to a blank page and the bug is not filed. ( I have also tried with other Return URLs as well, but to no avail.)

How do I make it work?
