Cannot veiw all tasks/ tasks are not saving properly(!?)

I've created a task for another administrator three times now... she nor I can find these tasks even when we try to view all tasks... it is not ALL tasks, in fact it's feeling more like some of the tasks some of the time...

Are we losing data?
