Collect data in forms & Print the form in prescribed formats

Dear Team,

Zoho has a facility to Print while Submit.  Going beyond, I would like to Print the Details collected in Templates by feeding the details as received from forms at appropriate places.

If Form has,
  1. Name: 
  2. Address:
  3. Phone
fields, then my template will have,

I, ____________ residing at ___________ having a phone__________________ wanted to do this thing.

Once I click submit, the information should come to me and the persons who have submitted should be able to get the PRINT in the template with their own name, address & phone no. For eg, if the Name is Vijay, Address is T.Nagar and Phone is 4433.  Then the result should come as

I, VIJAY residing at T.NAGAR having a phone 4433 wanted to do this thing.

Then get printed

Could you help me out with html coding for the same.
