Contact Form - Does not appear on published website

I have seen this a few times now. The contact form is there on the site editor, but on the published site it does not appear.

Have tried both Safari and Chrome browsers - but the same situation on both...no contact form!!!

Anyone have any ideas how to fix this? (it's very annoying).
