Contact form not rending correctly in IE9 & Helpful Tip

Hello,

I am having difficultly making the contact form render correctly in IE9.

Here is a screenshot of how the forms looks in Firefox, Chrome and Safari:


And here in IE9:


Any tips on how to correct this?

Additionally, the tip that I added to the email option shows as text, where as when I preview the form within Zoho, it shows as a question mark icon with roll over text. Any tips on how to implement this change as well?
