Created a form on Zoho Docs Spreadsheets - now it's dissapeared and can't revert back to an earlier version

Hello,

I created a Zoho spreadsheet form yesterday. It was working fine yesterday, but today I have discovered it has 'disapeared'. 

I have tried reverting back to earlier versions of the spreadsheet but cannot get hold of the form. I did not save all the questions so I really need access to it.
