I have created a simple app in creator for a simple form to hold and describe URLs. I have been able to link the app to CRM and I have a Module for the app where I can create entries. Where I am running into a problem is when I go to the Accounts module and try to add/create an entry from the Accounts module I get this error.

Any help with this would be greatly appreciated.
