CRM form not adding data or sending email.

CRM form not adding data or sending email.

I am new to Zoho and need some assistance.  I created the following form:  http://ctac-nh.zohosites.com/nick-test.html using embedded html.  The form does not update any data and does not send out an email when submitted.  It does got to the destination page as it should.  Any help determining what the issue is would be greatly appreciated.
