Custom Buttons Deactivated on Form

Custom Buttons Deactivated on Form

I have a form (TrialMapCrit) which is a stateless form that previously had no problems.  Now, however, the custom buttons are not working after anything on the form is selected (radio buttons).  I've tested it in Firefox and Safari but none of the other browsers.

Please advise.
