Hi, this is what I need to do.

Be able to have a custom leave form that is used when a person selects a particular leave type. This custom form will restrict the leave request to specific date and times for leave requests. (see below)

For instance

Employee requests Christmas shopping leave type, (we have a leave type of Christmas shopping set for 1/2 day)
Each staff is given .5 days for this leave type. We set expiry to only allow it in  the month of December

Employees can only request this 1/2 day on 
Dec 10 9am-12
Dec 11 1pm to 5pm
December 13 9am to 5pm

Can I clone the leave form to a custom form and place code for form validation to restrict these dates?

I tried cloning the leave form, but could not pick it from the list.

Maybe there is another way? I dont want to try and edit my leave form if possible.
