Custom Report Generating Duplicate Rows

I created a custom report that used the Notes - Title field. When I realized that was creating a separate row in the report for each note title, I removed that field from the reporting columns.

However, when I run the report I still get multiple rows showing up, even though it no longer reports note titles. This causes duplicate rows to be generated by the report.
