Duplicate entries in report drawing from two modules

I have created a report that pulls information from Leads (i.e. leads is the Module for which the new Report has to be created) and where the related module is Activity. When I run this report some leads are duplicated, and some with an activity appear 4 times.

Does anyone know whether there is an existing resolution for this or is it a bug. If the latter is a resolution scheduled?
