E-mailing a partially filled form

E-mailing a partially filled form

I need a way of e-mailing a potential client with a form that is pre-filled with the information that I already have for them and blanks for the information still needed in order to turn them from a lead to a client.

Is there a form that I can use to do this?  Suggestions on the easiest way to accomplish this task?
