Edit the Request Contract form?

Edit the Request Contract form?

I have Contract integration enabled with CRM and so have the 'Request Contract' button enabled in Deals.

Is it possible to edit the Request Contract Form that pops up when you select the 'Request Contract' button? 

Also, are the fields within the Request Contract Form available for use within the Contract template? If I use the 'Add Fields' button when creating a template they don't seem to be available. Ideally what I'd like to do is merge the 'Contract Effective Date' from the Request Contract Form into my Contract template,
