Edit this application problem - only one column of fields is now visible!

Edit this application problem - only one column of fields is now visible!

Just recently, when I click on "Edit this application" it only shows one column of fields available for edit.  This is only about 30% of the fields present in our database.  I need to edit these "missing fields."  Maybe I'm viewing it incorrectly?  Any solutions?
