form disappeared AGAIN!!!!!

OK. Whenever I make a minor change to a field the form disappears and is inaccessible. Is this going to happen every time I edit something? We need this form up ASAP.
Library Materials request form.
Please inform when we can get it back up, and what I can do to alleviate the problem in the future.
