Form Error

I tried to include a fill out form on my site. Everything looked fine except for the arrangement of the items. It appears that any new fields are listed on the bottom of the html coding.

I tried to insert a sentence within my code received this error when I test out the form to see if a lead was generated or not

"Sorry, the page you have requested was not found.

Now, the form worked before I tweaked the code and rearranged the layout of the questions. I tried to do this twice and received the same error message listed above. Does anybody know how I can insert a statement between my fields, while still allowing the form to automatically sync with my account?
