Form Is On Website, Used To Work, But Not Working Now

I can see it on my website . . . I submitted a test, and it is not coming through to my email address.  Also, I don't see the form when I go to webforms in the CRM.  I also thought I had set up an autoresponder for this form saying "thank you, we have received your request . . . etc".
