Form submissions not loading into CRM

I've used Zoho before and have setup the website form to CRM form before and it worked fine. So i needed to setup one on this new account as an easy way for me or someone else to submit new accounts into the system. However after submitting the form...nothing ever comes up in Zoho CRM. I'm not sure whats wrong...everything was done as I've done it before but for whatever reason this one will not work and its getting me behind getting accounts entered in.
