I can only create a new lead/account/contact etc from the Quick Create on the home page sidebar.

 

When I select from the top toolbar or try to edit an existing record, the form is not displayed. All that is shown is two identical lines of save, save & new, cancel.

 

I'm a new user so I may be missing something?
