Get image url from image uploaded to app

Get image url from image uploaded to app

I have a table in my app that holds uploaded images and a string value for each one.  I am trying to dynamically upload one of these images to a different form's image field based on a selection the user makes in that new form.  I need the entire image url to upload here, and I don't know how to get that via scripting.  Thank you.
