HELP PLEASE WITH CHECKBOX LIMITATION BETWEEN THE ZOHO FORMS in FORMS , CREATOR AND SITES

maybe i'm a novice but it's been frustrating using checklist in the 3 different ZOHO forms... from FORMS to CREATOR to SITES ... each has a function i need but none has the ability of all of which i need together.  what am i doing wrong with ZOHO SITES Form when i insert a checkbox field that it has no option for columns nor can i change it in the definition?  i want to display a list of 5 by 10 so 50 to choose from
