HELP!!!!! Simple Automation NOT working

Hi all!

I am trying to set up a simple workflow automation and it hasn't worked yet.

It is as follows:
Upon Create or Edit
if Lead Status is Approved OR Scheduled
Send Email Notification to the LEAD OWNER

That's it!  Please help me figure out where I am going wrong - none us as lead owners have gotten one notification! :)
