How can I add custom notes to an add customModule form?

I've created a customModule with the alias Contracts. Now I have the field 'duration (months)'.

I want to add a note (descriptive text) above or beneath with the text:

If contract is of indefinite duration, fill in 0 (zero)

How am I able to achieve this?
