How can I create a simple report to see the number of calls and emails sent to each lead in the database?

How can I create a simple report to see the number of calls and emails sent to each lead in the database?

I'm trying to create a report to know how many "touches" we've done with a lead (i.e, either sent them an email, made a phone call (inbound or outbound), or set up a meeting (event)).  I'm trying to setup a Matrix report, but when I select the Full Name as my column source, I cannot select Activity Type as the columns (which doesn't make sense to me).  When I setup a report to view the Lead name and Call Type (either inbound or outbound), I get a third column of "blank" which lists a lead even though there was no call type (or activity either), which doesn't make sense to me.

So if anyone can help with my confusion and give me a sample or tips on how to setup this type of report, I would appreciate it.  I would think this would be a standard report as you'd like to know if these leads are still worth pursuing or not (i.e., if you've called them and emailed them numerous times, chances are they aren't interested and you shouldn't keep reaching out to them).
