How can I push data from a Zoho Form field to the Notes (CRM)?

For instance, push data from a freeform text box in a Zoho Form, for example, "How did you hear about us" in the form to the Notes area of the CRM record. 

Is it possible? Please suggest way(s) to do it, if yes.
