How to Create Ticket Templates

Hi good day. I would like to ask an assistance on how we can create ticket templates / forms like these samples below. It would be better if they are in a "form" format instead of just filling up the description.


New Hire/Onboarding – this ticket type needs:


- Name: First & Last

- Preferred Name: ( sometimes people use a nickname or only a surname, etc )

- Address: ( needed for asset location and Endpoint controls )

- Office Location:  US, Brz, Ph

- Personal email: 

- Contact Number: ( needed for IT to contact – will not be added to M365 )


- Report to Manager: 

- Title:

- Department:

- Client:

- Start Date: 

- Type of Employee:  W2 Employee or Contractor

- Asset needs: Laptop or VDI ?

-Asset needs: Headset, Monitor, Dock, Mouse and Keyboard ?

- Software Requirements:

      1. Username

      2. email

      3. O365 with Teams

- Groups and Lists: ( we need to identify Teams to add them to, Security Groups and Distribution lists )

            ** Automatically add to Everyone, Brz, PH & US Groups

  

Equipment Request – this ticket type needs:

  • Name of Requestor:
  • Equipment Requested: ( laptop, monitor, headset, mouse, keyboard, webcam, other ? )
  • Equipment Assigned to: ( who is the equipment for ? )
  • Approver for Request: ( who needs to approve the request ? )

  

Employee Exit:

  • Name of Employee:
  • Requested by:
  • Last Day of Service:
  • Assets to Reclaim: ( what is being reclaimed and who is reclaiming it ? )
