How to embed web form on Facebook

Hi all:

I need to create a lead form in Facebook for people to request a product sample. I created my form and everything went fine, but I'm not getting the options to select embed, source, or iFrame. Also, I've seen another post on this forum where I should be getting tabs to select a specific supported site before selecting the code to use. I've attached a screenshot of what my final screen looks like. Any suggestions as to how to proceed?
