How to get Lookup list options to take values of multiple fields?

How can I make the names of the options of a Lookup checkbox list take the concatenated values of more than the usual one field of the looked-up form?

E.g. options

[ ] fred jones

[ ] mary smith

looked up from form Names, fields First and Second.
