How to pass websiteinfo to a form?

Hi,

I have a form with all fields identical. I have a hidden field at the top which indicates from which webpage the form is filled. So lets say the same form can be filled by companies, organizations, persons from 3 different relevant html page. Is there a way to address tis info to the hidden field on load or somehow.
