How to remove duplicate accounts?

Scenario - I am encountering problems with the accounts module. I have got the list of accounts lets to say 100 in accounts module I'm able to see the count as 100 but while creating the report it shows the count as 115.

After extracting the report it shows 15 as duplicates.

Can anybody help me out with removing these duplicates and why does this problem occur?
