How to upload attached file into CRM via survey

Hi there.....if a customer uses the "file upload", is there any way to get that into a field in the CRM Potentials module...even if it is just a link to the attached file?

There are ways to get all other text-based fields in there, but the only place I see the uploaded files are in the actual returned survey itself, which sits under the contact name, and NOT the potential name.

Since the survey I am sending out has to do with a specific potential, it will get confusing to have the images they upload under the contact....with surveys stacking up under the contact.
