Is this possible, Auto Send Form

I have a x number of past customers and leads who were last contacted 1 year ago, i was wondering if i could set up an automatic introduction email to be sent along with a notification to myself to follow up a call.  i know i would have to decide what criteria to have for this which i havent' looked into yet.
