I am having issues trying to organize the layout on this menu , I have tried editing the module and moving things around but the fields keep coming up like below.

I would like if i can get the fields in this order :

- Physician Name

- Physician Street

- Physician City

- Physician State

- Physician Zip

- Physician Tel

- Physician Fax

- NPI 

Rather than the order I seem to be stuck with now
