Lead capture info directed to Zoho

Currently we have a lead page embedded on our website that is linked to our current autoresponder service. (not Zoho)

When someone fills capture box their info gets dumped into current autoresponder service.

We are in the stages of migrating everything to Zoho but in stages.

How can we get our current lead page to also send the lead info into Zoho leads module?
