I want to create a survey that will have 2 purposes.

1.) Gather important feedback

2.) Gather contact information about the person who took the survey - example: First Name, Last Name, phone, email, etc...

Web to Lead Forms is NOT an options... because I have to create more questions than the allowed 150 custom fields and I do not want every single question I ask to be a custom field in the CRM (that not only doesn't make sense, it is not the best use of a custom field)

If I am using a Zoho product, I would expect that your products work together (that's the whole point of using one provider). So please tell me how I can gather their contact information, which will create a NEW LEAD in my CRM and show me a link within the CRM to their responses?

Zoho Surveys? Zoho Creator? - what and how do I accomplish this?
