Loss of Contact due to Lead Conversion

Loss of Contact due to Lead Conversion

We have a situation where a Lead was converted into an existing Account, but no corresponding Contact record was generated.  We have lost the Lead/Contact.  Has anyone else experienced this?  I have not been able to recreate the scenario.  Please advice.  Thank you in advance.
